Website Terms

Terms & Conditions

These Terms & Conditions apply to general use of the Summit Spine & Wellness website and certain related online features and communications.

The website is operated by Summit Spine and Wellness PLLC, doing business as Summit Spine & Wellness ("Summit," "we," "us," or "our").

By using this website, you acknowledge these Terms & Conditions.

These terms are intended to govern general website use only.

They are not intended to replace or modify any separate patient agreement, informed-consent document, financial policy, Notice of Privacy Practices, treatment agreement, communication authorization, or other document that may apply to healthcare services.

Where applicable law or a separate signed healthcare document provides additional or different rights or obligations, those requirements will control.

Website Information Is General Information

Information on this website is provided for general educational and informational purposes.

Website content may discuss topics such as:

  • Chiropractic care
  • Rehabilitation
  • Massage therapy
  • Recovery and wellness services
  • Digital X-ray
  • Movement and mobility
  • Insurance and payment options
  • Patient preparation
  • Summit services and office information

The information is not intended to provide an individualized diagnosis or treatment recommendation.

Healthcare decisions should be based on an appropriate evaluation of the individual patient.

Do not use information on this website as a substitute for care from a qualified healthcare professional who has evaluated your particular situation.

Website Use Does Not Automatically Create a Doctor-Patient Relationship

Viewing this website, submitting a general inquiry, calling the office, requesting an appointment, or communicating electronically with Summit does not by itself necessarily establish a doctor-patient relationship.

A clinical relationship generally begins only after appropriate patient registration, consent, evaluation, acceptance for care, and any other steps required by Summit or applicable law.

Information on this website should not be interpreted as a personal diagnosis, individualized treatment recommendation, or medical opinion from Dr. Spencer Hiebert regarding someone who has not been appropriately evaluated.

Medical Emergencies

This website, email, text messaging, and online scheduling systems are not designed for emergency medical communication.

If you believe you are experiencing a medical emergency, call 911 or seek appropriate emergency medical care.

Do not rely on:

  • Website forms
  • Ordinary email
  • Text messages
  • Appointment requests
  • Social media messages
  • General website communications

for urgent or emergency medical needs.

Individual Evaluation Matters

Information about a service or treatment does not mean that service will be appropriate for every person.

Recommendations may depend on factors including:

  • Health history
  • Examination findings
  • Symptoms
  • Age
  • Existing imaging
  • Previous injuries
  • Current activity
  • Goals
  • Contraindications
  • Other relevant clinical considerations

Clinical decisions are made on an individual basis.

No particular result can be guaranteed.

Healthcare and wellness outcomes may vary from person to person.

Online Appointment Scheduling

Summit may make online scheduling available through third-party systems, including ChiroTouch.

Submitting an appointment request or selecting a visit type does not necessarily guarantee:

  • Appointment availability
  • A particular provider
  • A specific appointment time
  • Eligibility for a particular service
  • Insurance coverage
  • Same-day treatment
  • A particular treatment
  • Clinical appropriateness of the selected service

Appointments remain subject to availability, scheduling requirements, eligibility, clinical judgment, and applicable Summit policies.

The Summit team may contact you if additional information is needed or if a different appointment type appears more appropriate.

SMS/Text Messaging Terms

Summit Spine & Wellness uses ChiroTouch to send patient-related text messages, including appointment reminders, scheduling updates, and other authorized communications.

Message frequency varies based on appointment activity and patient communication preferences. Message and data rates may apply.

Patients may opt out of supported automated text messages by replying STOP or contacting our office. Reply HELP for assistance where supported.

Consent to receive promotional text messages is not required to receive treatment or purchase services. Promotional messages, if offered, will require appropriate consent.

Depending on the communication, text messages may include:

  • Appointment reminders
  • Scheduling confirmations
  • Scheduling changes
  • Office updates related to an appointment
  • Requested patient communications
  • Other authorized administrative or patient-related communications

Standard text messaging may not always provide the same level of privacy or security as systems specifically designed for protected healthcare information.

Patients should avoid sending unnecessary sensitive medical information, medical records, insurance identification numbers, or other protected health information through standard text messaging unless directed to an appropriate secure process.

Opting out of promotional or other nonessential text messages does not prevent you from receiving treatment from Summit Spine & Wellness.

Where supported, replying STOP to an automated text will be treated as a request to stop applicable text messages. Patients may also contact the office directly regarding communication preferences.

Federal Communications Commission rules recognize reply-text methods such as STOP as a reasonable way to revoke consent to covered text messages.

For additional information about how Summit collects, uses, protects, and shares personal information, please review our Privacy Policy.

Privacy Policy:

Promotional Text Messages

If Summit offers promotional text messaging regarding special offers, wellness services, practice updates, or similar marketing communications, those messages will be handled separately from ordinary appointment and patient-care communications where required.

Consent to receive promotional text messages:

  • Is voluntary
  • Is not required to receive treatment
  • Is not required to purchase services
  • May be withdrawn as permitted by applicable law
  • May be subject to separate consent or opt-in requirements

Where promotional SMS consent is collected through a website form, the promotional consent option should be presented separately and should not be preselected on the patient's behalf.

Message frequency may vary.

Message and data rates may apply.

Reply STOP to opt out or HELP for assistance where supported.

Patient Intake & Health Information

Some scheduling and patient-intake processes may request personal, insurance, or health-related information.

When possible, sensitive health and insurance information should be submitted through Summit's designated patient systems rather than through general website forms, standard text messages, or ordinary email.

Information collected in connection with healthcare services may be subject to HIPAA and other applicable privacy laws.

Please review Summit's:

Privacy Policy

and applicable Notice of Privacy Practices for additional information.

ChiroTouch & Patient Communications

Summit Spine & Wellness uses ChiroTouch for functions that may include:

  • Electronic health records
  • Appointment scheduling
  • Patient intake
  • Appointment reminders
  • Patient communications
  • Other practice-management functions

Information submitted through or processed by ChiroTouch may be subject to additional privacy and security requirements applicable to healthcare information.

Where ChiroTouch or another vendor creates, receives, maintains, or transmits protected health information on Summit's behalf and HIPAA requires a business associate relationship, appropriate contractual protections are required.

HHS identifies electronic health record and practice-management vendors that handle PHI on behalf of a covered healthcare provider as examples of business associates subject to HIPAA requirements.

Minors

Summit may provide healthcare services to minors.

Parents or legal guardians may be responsible for completing appropriate scheduling, consent, intake, insurance, payment, and communication documentation for a minor, depending on applicable law and the circumstances.

Nothing on this website is intended to alter legal requirements concerning parental or guardian consent.

Insurance Information

Insurance information provided on this website is general in nature and may change.

Coverage depends on the individual insurance plan.

Summit may provide insurance benefit verification when possible, but verification is based on information available from the insurance carrier and should be treated as an estimate.

It is not a guarantee of:

  • Coverage
  • Payment
  • Network status
  • Final patient responsibility
  • Claim approval

Actual benefits may depend on:

  • Deductible
  • Copay
  • Coinsurance
  • Covered services
  • Visit limitations
  • Authorization requirements
  • Plan exclusions
  • Network participation
  • Claim processing
  • Other plan terms

The insurance plan makes the final determination regarding coverage and payment.

Learn About Insurance & Payment Options

Payment Responsibility

Patients may be financially responsible for charges not paid by insurance or otherwise covered under an applicable arrangement.

The exact financial responsibility may depend on:

  • Insurance coverage
  • Self-pay arrangements
  • Services received
  • Applicable financial policies
  • Other patient-specific circumstances

Accepted payment methods may include:

  • Visa
  • Mastercard
  • American Express
  • Discover
  • Eligible HSA cards
  • Eligible FSA cards
  • Cash
  • Checks

Some wellness and recovery services may be offered on a self-pay basis.

Pricing and payment information should be confirmed with Summit when it is important to a scheduling or treatment decision.

HSA & FSA Payments

Summit may accept HSA and FSA cards for eligible services.

Eligibility is determined by the applicable HSA or FSA plan and relevant tax or plan rules.

Summit does not determine whether every service qualifies under every individual plan.

Patients should confirm eligibility with their plan administrator when necessary.

Superbills & Possible Out-of-Network Reimbursement

For certain eligible self-pay services, Summit may provide an itemized receipt or superbill upon request.

A patient may choose to submit that documentation to an insurance company for possible out-of-network reimbursement.

Providing a superbill does not mean reimbursement will be available.

Any reimbursement depends on the individual insurance plan and how the insurer processes the claim.

Promotions & Special Offers

Summit may periodically make promotional offers available.

Promotions may be subject to:

  • Eligibility requirements
  • New-patient requirements
  • Self-pay restrictions
  • Expiration dates
  • Service limitations
  • Insurance restrictions
  • Federal healthcare program requirements
  • Clinical appropriateness
  • Other stated terms

A promotion does not determine what healthcare services are clinically appropriate.

If a service listed in an offer is not appropriate for a particular patient, Summit may decline to provide that service.

Specific written terms for a promotion will govern that promotion.

Consent to receive promotional SMS messages is not required to receive healthcare services or participate in an otherwise eligible offer unless applicable law permits such a requirement.

Wellness & Recovery Services

Summit may offer standalone wellness and recovery services, which may include:

  • Massage therapy
  • Infrared sauna
  • Red light therapy
  • Compression therapy
  • Other recovery services

These services may have their own:

  • Safety screening requirements
  • Preparation instructions
  • Eligibility requirements
  • Contraindications
  • Pricing
  • Scheduling requirements
  • Policies

Availability may change over time.

Some services may be available without a chiropractic examination, while others may require clinical evaluation.

Service Availability

Information on the website reflects services Summit expects or intends to make available.

However, a service being listed does not necessarily guarantee:

  • Immediate availability
  • Provider availability
  • Equipment availability
  • Appointment availability
  • Insurance coverage
  • Eligibility
  • Clinical appropriateness

Summit may modify, delay, suspend, or discontinue services as circumstances require.

Website Accuracy

Summit makes reasonable efforts to keep the website accurate and current.

However, information may change.

This may include:

  • Insurance participation
  • Pricing
  • Provider availability
  • Hours
  • Services
  • Office policies
  • Promotions
  • Equipment
  • Scheduling information

We cannot guarantee that every item on the website will always be complete or current.

When specific information is important to your decision, contact Summit to confirm it.

Website content may generally be viewed for personal, informational, and noncommercial use.

Summit works to maintain reasonable website availability.

However, websites and third-party systems may occasionally experience:

  • Maintenance
  • Technical errors
  • Internet outages
  • Security issues
  • Third-party service interruptions
  • Other circumstances outside Summit's control

Summit does not guarantee uninterrupted website access.

If an online feature is unavailable, contact the office by phone when appropriate.

Website Security

Summit uses reasonable safeguards appropriate to the systems it operates.

However, no internet-based service can be guaranteed to be completely secure.

Users should avoid transmitting unnecessary sensitive health, financial, or personal information through general website forms, standard text messages, or ordinary email when a designated secure process is available.

No Guarantee Regarding Website Information

To the extent permitted by applicable law, Summit provides general public website content for informational purposes without guaranteeing that the information:

  • Will apply to every individual

Severability

If a provision of these Terms & Conditions is determined to be invalid or unenforceable, the remaining provisions may continue to apply to the extent permitted by law.
